Job summary
Nexus Family Healing is expanding with a new youth crisis and stabilization center in Hennepin County. The Admissions and Transitions Coordinator will coordinate therapeutic services and community support to ensure stabilization and successful discharge planning for youth with complex mental health needs.
The role is a full-time hybrid position based in Minneapolis, with duties overlapping office and community settings, and a pay range of $22-$24 hourly, plus comprehensive benefits including
Qualifications
- Bachelor’s degree in social work or related field.
- 5+ years of experience in social work or case management.
- MN Mental Health Practitioner definition compliance.
- Valid driver’s license required.
Responsibilities
- Coordinate ongoing therapeutic services and community supports for stabilization care.
- Link clients to services and assist in discharge planning.
- Develop stabilization and discharge plans with teams.
- Provide crisis intervention and family consultation as needed.
- Maintain documentation and communicate with supervision and partners.
