Job description

The Department of Mental Health (DMH) is seeking a qualified professional for the position of Adult Case Management Supervisor (Human Service Coordinator II) at the Corrigan Mental Health Center to provide direct supervision to staff who work with adults in the community or hospital settings.

Please Note : This is a full-time 40 hours per week position. The schedule for this position is 8:30 AM to 5:00 PM, Monday through Friday. The days off will be Sunday and Saturday.

Duties and Responsibilities (these duties are a general summary and not all inclusive):

  • Provides direct case supervision and consultation to HSC Is by meeting on a regular basis, reviewing documentation as needed and providing consultation on difficult client issues in order to assure quality service provision.
  • Demonstrates the knowledge and skills necessary to provide appropriate care for the age-related and cultural needs of the clients served.
  • Manages case management resources through assignment/monitoring caseloads and follows up with authorized individuals awaiting available DMH services.
  • Provides direct case supervision and consultation to the HSC I TAY supporting continuity of care for transitional age youth.
  • Ensures implementation of Case Management regulations, policies, and procedures, especially Individual Service Plan regulations and standards, to meet timelines in compliance with established standards of continuing care services.
  • Coordinates, monitors, and assesses the quality and quantity of Case Management reviews; reviews and countersigns Comprehensive Assessments, and reviews ISPs required by Department regulations.
  • Conducts and completes Employee Performance Reviews within prescribed timelines.
  • Acts as one of the Site’s Needs & Means Specialist in the completion of the Authorization Process by receiving the clinical record, assessing the need for services, responding to requests for interim services, and by providing direct support and case management services when required.
  • Assists the Clinical Director in monitoring the Respite and vendor ACCS contract by reviewing vouchers for accuracy, reviewing, and approving EIM submissions by providers, meeting with providers, identifying performance problems and offering assistance to providers in order to assure effective and efficient service delivery.
  • Negotiates the assignment of targeted case management supports to assure advocacy on behalf of hospitalized/respited clients, to assist in the securing and confirmation of timely outpatient aftercare appointments, and to assure that required treatment and medications are provided prior to discharge.
  • Facilitates inter-intra agency coordination and cooperation in order to resolve referral problems and participates in planning and policy development.
  • Participates on assigned committees for the purpose of program development and coordination.
  • Comply with all mandatory training requirements.
  • Performs related duties as required.


Preferred Qualifications:

  • Bi-lingual preferred.
  • Strong and effective interpersonal skills, including the ability to actively listen, relate to others and demonstrate sensitivity and respect for individuals.
  • Proficiency in Microsoft Office applications including Outlook, Word, and Excel.
  • Previous case management experience.
  • Previous experience working with people with chronic mental illness and/or substance use disorder(s).

Based on assignment as a Mental Health Case Manager or Forensic Transition Team Coordinator, within the Department of Mental Health, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation counseling, education or other a related field is required.

In addition, applicants must have at least four (4) years of full-time, or equivalent part-time, case management of clinical/therapeutic experience.

Based on assignment, travel may be required. Incumbents who elect to use a motor vehicle for travel must have a current and valid motor vehicle driver's license at a class level specific to assignment.

Salary placement is determined by a combination of factors, including the candidate’s years of directly related experience and education, and alignment with our internal compensation structure as set forth by the Human Resources Division’s Hiring Guidelines. For all bargaining unit positions (non-management), compensation is subject to the salary provisions outlined in the applicable collective bargaining agreement and will apply to placement within the appropriate salary range.
