Job description

The Department of Developmental Services (DDS) is seeking candidates for a Human Services Coordinator II in the Northeast Regional Office for the Acquired Brain Injury program known as the ABI/MFP (Acquired Brain Injury / Moving Forward Plan) Program in Danvers, MA. The Human Service Coordinator II position provides supervision, support, consultation, and orientation to assigned Human Service Coordinators. The incumbent will help coordinate and facilitate the development of Individual Support Plans (ISPs). The HSC II will function as the Service Coordinator Supervisor for the ABI/MFP program.

Job Responsibilities: (The duties and responsibilities stated are general summary and not all inclusive.)

  • Responsible for providing direct supervision to at least five Service Coordinatorson the Regional ABI/MFP team. Support and monitor the Human Service Coordinators performance and ensure compliance with statewide guidelines.
  • Serves as a core member of the ABI/MFP leadership team. Participates in planning, policy development and resource allocation activity.
  • Collaborates with ABI/MFP leadership team on newly eligible participants, residential referrals, and complex clinical and medical cases.
  • Partners with regional Skilled Nursing Facilities and ABI/MFP team members to transition individuals served by the waiver to the community.
  • Assigns and monitors cases toHuman Service Coordinators; serves as liaison for individuals entering the ABI/ MFP waiver system and compiles, maintains and prepares individual’s statistics and resource service data.
  • Facilitates collaboration and discharge planning with UMASS Medical School, Provider Network, MassAbility, Skilled Nursing Facilities, MFP Demonstration Projects Office, and Transition Entities.
  • Supports Service Coordinators to involve regional resources: risk management, human rights, neuropsychological, psychological, medical, OT/PT/SLP, DDS legal, etc.
  • Oversees the service coordinator’s monitoring of the implementation of individual services provided and maintains regular and consistent contact with individual/guardians, service providers and families as indicated.
  • Oversees the maintenance of individual records and ensures complete and accurate information including compilation and preparation of case statistics and resource/service data.
  • Oversees special projects as assigned and coordinates and facilitates the development of an Individual Support Plan that incorporates a broad spectrum of individual needs, skills and desires.
  • Participates in training role, including field inter-agency and provider training.

Preferred Qualifications:

  • A deep understanding of developmental disabilities, specifically individuals with acquired brain injury, and have a proven track record of working with individuals with disabilities and their families.
  • Ability to provide leadership to staff and non-staff; be able to work successfully in a complex and evolving human service system while maintaining a clear sense of purpose and commitment.
  • Able to relate to a diverse work force and work successfully with consumers representing diverse needs and cultures.
  • Familiarity with ABI/MFP waivers and have an understanding of the various MassHealth and UMASS policies and procedures which have bearing on DDS consumers participating in the waiver programs, as well as the documentation and data entry requirements of DDS including Home and Community Services Information System (HCSIS) and MEDITECH.
  • This position will directly supervise a team of at least five Service Coordinators. Experience in supervising or administering services for adults with disabilities, specifically individuals with acquired brain injury, is essential.

  • This position is located in Danvers, MA
  • Full-Time, 37.5 hours per week.Schedule: 8:45 AM-5:00 PM Monday-Friday. Eligible for a hybrid schedule.Days Off : Saturday/Sunday.

Applicants must have (A) at least four (4) years of full-time, or equivalent part-time, case management or clinical/therapeutic experience (B) any equivalent combination of the required experience and the substitutions below.

Based on assignment, travel may be required. Incumbents who elect to use a motor vehicle for travel must have a current and valid motor vehicle driver's license at a class level specific to assignment.

Substitutions:

I. A Bachelor's degree or higher in a related field may substitute for two (2) years of the required (A) experience.

Based on assignment as Qualified Intellectual Disabled Professionals, within the Department of Developmental Services, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, physical education or other a related field is required.

Salary placement is determined by a combination of factors, including the candidate’s years of directly related experience and education, and alignment with our internal compensation structure as set forth by the Human Resources Division’s Hiring Guidelines. For all bargaining unit positions (non-management), compensation is subject to the salary provisions outlined in the applicable collective bargaining agreement and will apply to placement within the appropriate salary range
