Overview
The Human Services Case manager willprovide case management for the Trenton Citadel social service programs under the direction of the Corps Officer,with professional supervision provided by Divisional Headquarters Human Services Staff. TheCase Managerwill be able to identify the needs of the clients served and provide outreach within the community at large.
Responsibilities

Specifically, the primary duties are:
- Perform intakes, interviews, and assessments of clients’ needs and perform follow up case management with households for other supportive programs available if needed beyond emergencyassistance.
- Identifyand interact with case management clients; provide comprehensive case management including but not limited to assessment of client needs, developing a plan of actionutilizinga strengths-based approach, andidentifyinginternal and external programs that would support their pathway to self-sufficiency.
- Coordinate services with partner agencies to ensure a holistic service approach for all clients.
- Support the Pathway of Hope Initiative by enrolling eligible families with children under 18 who are living in generational/chronic poverty and providing the above case management in conjunction with the Pathway of Hope policies and procedures.
- Ensure the accuracy andtimelysubmission of records and reports in applicable databases for all community programs and funding sources as requested by the Corps Officers.
- Maintainaccurate,confidential,and updated case recordsin accordance withThe Salvation Army record keeping policies and procedures.
- Placeall food orders,submitreports on time, organize andmanageinventory,and handleall paperwork from the local Food Bank.
- Monitor andmaintainthe general condition and upkeep of Social Service lobby and reception area.
- Manage the main switchboard,which includes but is not limited tomaking phone calls, taking messages,and listening to voicemails to ensure social services clients are receiving assistancein a timely manner.
- Assist Corps Officers in overseeing volunteers and community service workers.
- Prepare andmaintainSalvation Army statistical reports fortimelysubmission.
- Attend andparticipatein regular staff meetings/trainings and individual supervision meetings asrequired.
- Uphold the standards and principles of The Salvation Army’s mission and ministry.
- Other relevant duties as assigned by supervisors.
Qualifications
- Associate’s DegreeRequired in Social Work, Psychology, Counseling or Human Servicesrelated field from an accreditedinstitution.
- Master’s Degree in Social Workpreferred.
- At least 1 to 2 years of experience in case management or counseling.
- Strong written and verbal communication skills
- Bilingual (English/Spanish)required.
- Possess organizational skillsandprofessional demeanor.
- Knowledge of Microsoft Office programs, Word, Excel, Access.
- Knowledgeableof Mercer County and community resources.
- Strong interpersonal skills and ability to work as part of a team.
- Excellent written and verbal communication skills, ability toestablishrapport.
- Ability tomaintainconfidentiality of interactions with clients and fellow employees.
- Awareness and sensitivity to cultural and socioeconomic characteristics of populations served.
- Good organization skills andtimelyfollow up skills to ensure that any concerns are reportedtimely, clearly, and addressed inan appropriate timeframe for overall cleanliness and safety.
