The Salvation Army Northern Division Harbor Light Center (HLC) seeks a dedicated Case Manager to provide comprehensive, person-centered services to adults. You will support housing, employment, and self-sufficiency goals, while maintaining precise records and promoting community resources.
Responsibilities include developing individualized plans, recruiting participants, and promoting HLC initiatives through outreach and collaboration with partners.
TheSalvation Army Northern Division Harbor Light Center (HLC) and satellite locations is Doing the Most Good by providing shelter, housing, and holistic, person-centered, supportive services to over 300 single adults each night. HLC also offersworship services, pastoral care, and connections tocommunity partners.
The Case Manager provides individualized case management to supportadultparticipants in achieving housing, employment, and self-sufficiency goals. This roleis responsible formaintainingaccurateparticipant records, recruiting eligible participants, and promoting social services programs. Additionally, the Case Manager engages in community outreach, supports organizational initiatives, andfacilitateseducational opportunities to connect participants with valuable resources.
