Job summary

Beaufort County seeks a Human Service Analyst to provide care coordination services, assisting eligible clients with access to medical, social, psychosocial, educational, financial, and other services for health-related needs to maximize independent functioning in the community. The role includes assessment, planning, problem solving, connection to services, and family training for those under age 6.

A bachelor’s degree and one year of relevant experience are required, with credentials (BabyNet)

Qualifications

  • Bachelor’s degree from nationally recognized educational accrediting body.
  • One year working experience with target population or an equivalent combination of education, training, and experience.
  • Documents experience, skills, or training in crisis intervention, effective communication, and cultural diversity and competency.
  • Knowledge of community resources.
  • Working knowledge of families and/or system of theory.

Responsibilities

  • Completes intake forms and procedures for clients applying for assistance.
  • Schedules appointments and meetings for potential clients.
  • Develops and maintains individual family service plans which includes developing goals based on the family's priorities and needs.
  • Conducts routine based interviews which is an assessment of the family's daily routines, priorities, supports, and concerns.
  • Conducts curriculum-based assessments to assess child developmental levels and progress.
  • Provides family training and parent coaching. Schedules and completes home visits by modeling appropriate interactions with family and child that have been created for the individual family and child.
  • Coordinates therapy services and resources for families by consulting with other agencies and providing referrals for individuals.
  • Maintains multiple state level data systems with detailed documentation of services rendered on behalf of the families and keeping the data systems up to date with current family/medical/financial information as well as maintaining a physical record.
  • Monitor progress, service delivery, and family satisfaction with early intervention services.
  • Receives and verifies accuracy of all documents, forms and applications provided by clients for completion and ensures program eligibility.

Description

The purpose of this position is to provide services, functions, and activities of care coordination which assist eligible clients with access to needed medical, social, psychosocial, educational, financial, and other services for health-related needs required to maximalize independent functioning in the community. The Human Service Analyst will complete assessment, planning, problem solving, and identification of and connection to any needed services in addition if needed provide Family training to those under the age 6.

Examples of Duties

The essential functions listed below are those that represent the majority of the time spent working in this class. Management may assign additional functions related to the type of work of the class as necessary.

Typical Qualifications
MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:
  • Requires Bachelor’s degree from nationally recognized educational accrediting body.
  • One year working experience with target population or an equivalent combination of education, training, and experience.
  • Documents experience, skills, or training in crisis intervention, effective communication, and cultural diversity and competency.
  • Knowledge of community resources.
  • Working knowledge of families and/or system of theory.
Special Certifications And Licenses
  • Within 90 days of hire, obtain valid South Carolina Part C BabyNet Credentials.
  • Infant and Child First Aid.
  • Adult and infant CPR.
  • OSHA.
  • Aegis.
  • Must possess and maintain a valid state driver’s license with an acceptable driving history.
  • IMPORTANT: Submit a copy of your certifications with application. Applications submitted without these items will not be considered.
Supplemental Information

All applicants tentatively selected for a position with Beaufort County Council will be required to submit to urinalysis to screen for the illegal use of drugs and to a background check prior to appointment. Employment is contingent upon clearing both the drug screening and background investigation.
