Job summary

Qualifications

• Master's degree in social work from a CSWE-accredited program.

• English language proficiency for appointment.

Responsibilities

• Provide case-management-based social work services to HUD-VASH Veterans.
• Serve as liaison with the local Public Housing Authority and VA providers.
• Coordinate care with community health and mental health services and housing agencies.
• Assess Veterans' needs and formulate/implement treatment plans.
• Maintain clinical notes and comply with licensure requirements.
