Responsibilities

• Provide comprehensive clinical case management for Veterans enrolled in HUD‑VASH.
• Collaborate with interdisciplinary teams to develop and implement housing stabilization plans.
• Coordinate services with landlords, housing authorities, VA programs and community partners.
• Deliver veteran-centered, trauma-informed, recovery-oriented care and crisis intervention when needed.
• Communicate effectively with Veterans, families, and community agencies to support housing tenure and recovery.
