Veterans Health Administration, Department of Veterans Affairs, seeks an experienced Registered Nurse for HUD/VASH Case Management. You will coordinate medical and psychosocial services, manage complex housing and health needs, and collaborate with VA and community partners to optimize Veterans’ care.

The role emphasizes leadership in care coordination, adherence to VA standards, and comprehensive assessments to support housing stability and well-being of Veterans in the HUD/VASH program.

Qualifications

English proficiency required for direct patient care.
Graduate of an approved nursing program and eligible for licensure.
Licensure and registration requirements as per VA standards.
Foreign nursing education requires degree equivalency validation.

Responsibilities

Provide patient management and care coordination in HUD/VASH.
Coordinate with VA and community providers for comprehensive services.
Assist in discharge planning and case management within HUD/VASH.
Ensure Veterans meet program eligibility and housing placement goals.
Maintain program documentation and outcome measures.
Travel to HUD/VASH sites as required by program duties.

Skills

Mental Health experience
