Job summary

YouthLink MN in Minneapolis is seeking a Case Manager – Scattered Site Housing to support youth aged 18-24 who are homeless or precariously housed. You will coordinate wrap-around services, referrals for housing, counseling, financial guidance, job readiness, and higher education in collaboration with county and city partners.

The role requires 1 year of case management, 2 years with at-risk homeless youth, and a valid MN driver’s license.

Qualifications

• 1 year of case management experience.
• 2 years’ work with at-risk, unsheltered youth, who may have some county involvement, criminal justice, probation officer, county social workers, or previously aged out of foster care.
• 2 years’ experience working with homeless youth.
• 2 years’ experience with community engagement.
• Must be able to come to work on time, for all regularly scheduled work shifts.
• Valid MN State Driver’s License, insurance.
• Knowledge of HUD, CES, HMIS, and City of Minneapolis housing regulations.
• Experience with client data tracking systems, working with landlords, running rental history reports (RHR), and working to get young adults with criminal records into stable housing.

Responsibilities

• Provide wrap-around case management for youth ages 18-24 who have been systems-involved.
• Coordinate housing referrals, counseling, financial counseling, job readiness programs, and higher education services.
• Collaborate with Hennepin County In Reach staff, the Director of Housing, and the Finance team.
• Maintain confidentiality and professional standards while engaging youth and families.
• Utilize HUD, CES and HMIS knowledge to support housing processes.

The Case Manager – Scattered Site Housing provides support, guidance, and coordination of wrap around services for youth ages 18-24 who have been systems-involved (corrections, foster care) with the goal of stabilizing them in housing. Working in collaboration with Hennepin County In Reach staff, the Director of Housing and the Finance team, the Case Manager-Scattered Site Housing assists youth with obtaining self-sufficiency through a holistic approach, including but not limited to appropriate referrals for housing, counseling, financial counseling, job readiness programs, and higher education.

Requirements

Required Education, Experience, Licensure, Certification required for entry into position:

• 1 year of case management experience.
• 2 years’ work experience with at-risk, unsheltered youth, who may have some county involvement, criminal justice, probation officer, county social workers, or previously aged out of foster care.
• 2 years’ experience working with homeless youth.
• 2 years’ experience with community engagement.
• Must be able to come to work on time, for all regularly scheduled work shifts

Knowledge, Skills, and Abilities and or Competencies required to successfully perform the work:

• Comfort with Microsoft Office applications as well as exhibit an ability to learn and utilize handheld applications systems and other electronic recording systems.
• Experience and knowledge with Coordinated Entry referrals and working with similar systems and housing programs within the County.

• Demonstrate an ability to utilize continuous improvement techniques and quality research methods to optimize overall program operation and build community relationships.
