Job summary

House of Hope Green Bay is seeking a dedicated Case Manager to oversee client services in Housing Stability programs. You will supervise interns, ensure program delivery aligns with licensing and grant requirements, and direct staff in providing comprehensive support to clients.

The role involves coordinating intake, developing and monitoring case plans, and maintaining compliant records, while contributing to outcomes and community engagement.

Qualifications

• Valid Wisconsin driver's license (or ability to obtain)
• Experience with direct services in housing or human services preferred
• Ability to work effectively with diverse, multicultural clientele
• Strong organizational skills and attention to record-keeping
• Ability to work flexible hours and as part of a team

Responsibilities

• Provide case management, housing navigation, and case plan oversight.
• Coordinate with local agency caseworkers.
• Maintain accurate client records and case notes in systems.
• Assist residents with case plan goals and crisis intervention when needed.
• Support program outcomes measurement and reporting.
• Participate in community trainings and meetings.
