Job summary

Beacon Hill in Boston, Massachusetts seeks a Stabilization Case Manager to join the Support Services Team. The role covers outreach, referrals, intake and assessment, case management, rapid housing, and housing stabilization.

You will develop relationships with landlords and property managers to secure permanent housing, coordinate with community partners, and support clients toward growth and stability. Schedule is Monday to Friday, 40 hours weekly, with standard daytime hours.

Qualifications

• Bachelor’s degree in Human Services or related field.
• 1+ years community case management experience.
• Proficiency in Harm Reduction, Motivational Interviewing and Trauma-Informed Care.
• Valid Driver's license.

Responsibilities

• Work with all aspects of the program: outreach, community referrals, intake and assessment, case management, rapid housing and housing stabilization.
• Develop relationships with landlords and property managers for permanent housing placement and retention.
