Job summary

HARRISONBURG ROCKINGHAM CSB in Harrisonburg, VA seeks a Service Coordination Specialist to support individuals with serious mental illness and co-occurring disorders through the Permanent Supportive Housing program. Qualifications

• Knowledge of community resources and housing-first models; ability to work with diverse populations.
• Strong communication, problem-solving, and crisis management skills; ability to handle confidential information.
• Experience in case management, social services and/or supportive housing preferred.
• Basic computer literacy.
• Must lift up to 25 lbs.

Responsibilities

• Conduct assessments to determine individual needs and goals.
• Develop and implement individual service plans; regularly assess progress.
• Provide ongoing support to maintain housing stability.
• Assist with daily living activities including budgeting, appointments, transportation to food banks and healthcare.
• Link participants to community-based services: mental health, substance use treatment, medical care, employment, and legal aid.
• Serve as the primary contact for housing staff and service providers.
• Monitor connections to services and advocate as needed.
• Address crises and emergencies; respond to eviction threats.
• Support move-in/move-out processes; access to crisis services.
• Advocate for benefits, resources, or legal protections.
• Documentation and reporting; maintain client records per requirements; complete housing stabilization plan, ISP, and notes.
• Track progress and coordinate with PSH team; backup to Case Manager and Housing Specialists.
• Participate in SOAR training; conduct annual apartment inspections for HUD quality standards.
• Engage in ongoing professional development.
• Participate in treatment team meetings; ensure Housing First adherence.
• Use evidence-based practices (Motivational Interviewing, Harm Reduction, Trauma-Informed Care).
• Utilize technology and maintain professional relationships.
• Perform other duties as agency needs require.
