Job summary

Illumination Health + Home (IH+H) is a growing nonprofit dedicated to disrupting homelessness through targeted services across recuperative care centers, shelters and housing programs in California. The Housing Retention Specialist helps clients maintain housing by addressing barriers, coordinating resources, and advocating for stability.

Role hours are Monday–Friday, 8:30am–5:00pm, with a pay range of $23.00–$26.00 per hour.

Qualifications

• Experience providing case management to individuals or families experiencing homelessness.
• Knowledge of HIPAA and client confidentiality standards.
• Ability to coordinate with landlords and community resources.
• Strong advocacy and liaison skills.

Responsibilities

• Manage a caseload of 30-35 clients.
• Collaborate with internal and external partners to promote retention outcomes.
• Advocate for clients and connect to housing stabilization resources.
• Coordinate charting and documentation per policy.
• Attend interdisciplinary care team meetings.
• Maintain client records in a HIPAA-compliant manner.
• Provide life skills training on tenant rights and maintenance.
• Coordinate with landlords to address concerns and negotiate payments.
