Community Bridges, Inc. is seeking a Housing Specialist to join the F/ACT team in Arizona.
You will implement permanent supportive housing, coordinate with agencies for housing opportunities, and provide in-field client support while working with landlords to ensure safe, suitable placements. The role requires a HS diploma (associates preferred), 1–3 years in behavioral health, a valid AZ driver's license, and relevant certifications (SOAR, VI-SPDAT preferred).

The F/ACT Housing Specialist works with a multidisciplinary team to implement the permanent supportive housing model with members assigned to their respective F/ACT team. The F/ACT Housing Specialist is expected to work with community supportive housing agencies on temporary and permanent housing opportunities for their clients. The F/ACT Housing Specialist will provide in field support to clients and work with landlords to ensure ongoing safety to the client as well as the appropriateness of the placement.
