Job summary

InterCommunity, Inc. is a Federally Qualified Health Center Look-Alike delivering compassionate care across multiple centers in Connecticut.

The role focuses on managing care for individuals with mental health or addiction disorders within a supervised housing setting, coordinating services, and promoting independence. The position requires a health-care degree and case-management experience in behavioral health, with potential substitutions for advanced credentials.

Qualifications

• Bachelor’s degree in a health care field and two years of case management experience in Behavioral Health.
• Master’s degree in a behavioral health discipline may substitute for one year of general experience.
• A current Connecticut RN license with two years of psychiatric setting experience may substitute for the bachelor’s degree.

Responsibilities

• Implements programs and monitors clients within a supported/supervised living program.
• Functions as a member of an interdisciplinary team and participates in service delivery plan processes.
• Provides guidance to develop independent living skills among clients.
• Assists clients with activities of daily living and access to needed services.
• Maintains health and safety standards and documents client progress.

Summary

Accountable for acting as part of a team in the service delivery plan for persons with mental health and or addiction disorders and the care, guidance, supervision and training of clients in a supported or supervised housing program.

Essential Duties & Responsibilities

• Implements programs and monitors clients within a supported/supervised living program
• Functions as a member of an interdisciplinary team and participates in service delivery plan process for clients
• Provides guidance and support for clients to develop independent living skills
• Participates in and assists clients with activities of daily living
• Assists clients in accessing goods, services and supports required for independent living; reinforces appropriate client behavior through modification techniques
• Responsible for clients health and safety
• Observes and reports client behavior
• Serves as advocate for government subsidies and re-determination hearings
• Transports clients
• Acts as liaison to clients and their families or various community agencies or organizations
• Provides concise and accurate reports and documentation
• May make recommendations on policies or standards
• May administer and track medications with appropriate certification
• May administer first aid in emergency situations
• May schedule and attend appointments with clients
• Performs related duties as required

*All agency staff are required to attend all mandatory department/agency meetings and trainings*

Sunday - Thursday, 8:00 AM - 4:30 PM

Requirements

Education &/Or Experience

A Bachelor’s degree in a health care field and two (2) years of experience using case management techniques in a Behavioral Health setting. A Master’s degree in a behavioral health discipline may be substituted for one (1) year of the General Experience. A current Connecticut license as a registered professional nurse and two (2) years of experience working in a psychiatric setting may be substituted for the Bachelor’s degree. On occasion, extensive community mental health experience with seriously and persistently mentally ill persons may be substituted for the degree requirements noted above.
