Job summary

HomeFirst in California seeks a Case Manager-Floater for the Housing Retention program. You will provide client-centered case management across multiple sites, including North County, Markham Plaza, Iamesi Village, EHV, Mesa Terrace and Solaire Apt locations in Santa Clara County.

You will coordinate services with staff and partners, document in HMIS within three business days, and assist clients with transportation and life skills development to promote stability and independence.

Qualifications

• 2 years paid or volunteer experience in social work or related human services required.
• Lived experience encouraged to apply.
• BA/BS in Social Work or related field preferred, not required.

Responsibilities

• Provide case management and other direct services to clients across multiple program sites with flexible on-site presence.
• Coordinate with team members remotely and on-site to communicate interventions.
• Collaborate with community partners to ensure access to support services.
• Maintain HMIS documentation within three business days and complete related paperwork.
• Assist clients with transportation to critical appointments using the company vehicle.
