Job summary

Chestnut Health Systems is seeking a Housing Clinician to provide recovery-oriented, trauma-informed mental health services to individuals in housing programs, delivering care in office and community-based settings. The role emphasizes assessments, treatment planning, and coordination with housing and substance use recovery partners.

This part-time position works Fridays from 4:00pm to 9:00pm at the Madison location, with responsibilities including documentation, pre-authorization tasks, and

Qualifications

• High school diploma or equivalent; five years' social service or peer experience, or CRSS/CPRS; or bachelor’s degree.
• Valid driver’s license required.
• Basic computer proficiency (Outlook, MS Word, EMR).

Responsibilities

• Complete all required documentation (assessments, treatment/recovery plans, progress notes, and funder paperwork).
• Ensure 100% of assessments and treatment/recovery plans are completed before due dates.
• Submit at least 80% of progress notes within 48 hours of services.
• Provide services based on individualized treatment, recovery, or safety plans.
• Meet assigned productivity expectations.
• Complete service pre-authorizations and re-authorizations on time.
• Conduct screenings for new housing referrals when assigned.
• Promote a recovery-focused, trauma-informed care approach.
• Connect consumers to community resources and supports.
• Attend team meetings, consumer staffings, and program development activities.
• Provide backup coverage when needed.
• Attend and document required supervision (Rule 132, if not LPHA).
• Complete Chestnut trainings and CEUs; take on leadership responsibilities as assigned.
• Use evidence-based practices and deliver services in community or natural environments.
