Job summary

Chestnut Health Systems seeks a Housing Clinician to deliver recovery-oriented mental health services to individuals in housing programs. Work is part-time, primarily Fridays 4:00pm to 9:00pm at our Madison, IL location, with office and community-based duties focusing on stability, wellness, and independence.

Qualifications

High school diploma or equivalent with five years of social service or peer experience.
Or CRSS/CPRS certification.
Or a bachelor’s degree.

Responsibilities

Provide recovery-oriented, trauma-informed mental health services.
Complete assessments, treatment plans, progress notes, and funder paperwork.
Ensure assessments and plans are completed by due dates.
Submit progress notes within 48 hours of service.
Deliver services based on individualized treatment, recovery, or safety plans.
Meet productivity expectations and complete pre-authorizations on time.
Connect consumers to community resources, including housing supports.
Attend team meetings and supervision, and participate in program development.
