Job summary

Ibis Healthcare is seeking a Care Manager for Community Housing to provide case management for clients in the homeless program who may have chemical dependency or co‑occurring disorders. The CHSC helps individuals move into housing and recover with a paycheck of their own.

Responsibilities include intake, assessments, developing management plans, transitions and discharge planning, and collaboration with community partners.

Qualifications

• Read, analyze, and interpret clinical records, procedures, or governmental regulations.
• Write reports and document clinical activities; communicate with managers, patients, and stakeholders.
• Knowledge of homeless services: 12-step, behavior modification, case management, and addiction.

Responsibilities

• Conduct client intake, screening, orientation, assessments, and crisis intervention.
• Develop and implement case management plans addressing housing, employment, and behavioral health goals.
• Develop transitions and discharge plans including housing, employment, and behavioral health provisions.
• Provide case management services and participate in client staffing and team meetings.
• Facilitate life skills groups with fidelity to evidence-based practices.
• Prepare and maintain clinical records within required timeframes.
• Interface with community partners and document referrals/outcomes.
• Assist in training and education of staff, interns, and students.
