Job summary

Union Station Homeless Services seeks a dedicated Peer Outreach Specialist to conduct street and encampment outreach, engaging chronically homeless individuals in the housing process. You will be part of a Multidisciplinary Team that coordinates housing readiness, services access, and case management planning.

The role emphasizes linking clients to mental health, health services, employment resources, and housing options, with data entry and on-site transportation support as needed.

Qualifications

• Experience with homelessness and housing needs are preferred.
• Knowledge of local housing resources in Los Angeles County and San Gabriel Valley is beneficial.
• Familiarity with HUD guidelines and subsidized housing programs preferred.
• Strong verbal and written communication skills are desirable.
• Proficient with basic computer use and data entry in HMIS or similar systems.

Responsibilities

• Conducts outreach with chronically homeless individuals to pursue permanent housing options.
• Receives and triages referrals from walk-ins and partners.
• Triages chronically homeless individuals within a multidisciplinary outreach team.
• Assists individuals with collecting documents for permanent supportive housing.
• Coordinates services with other providers at case conferences.
• Connects individuals to mental health, health, and employment resources as needed.
• Documents services and enters data into HMIS or similar systems.
• Provides transportation assistance to stabilize housing status.
• Assists with accessing permanent housing opportunities and move-in services.
• Performs other duties as assigned.
