Job summary

Clinical and Support Options, Inc. is seeking a Housing Navigator for our shelter in Greenfield. The role provides case management to help clients obtain and maintain housing, develop housing plans, and support successful transitions.

Starting salary is $38,000-$44,000/year; full-time, first shift with weekend options may apply. Responsibilities include intake assessments, housing plan development, caseload management, collaboration with FOH shelter guests, and documentation in CSO’s systems.

Qualifications

• College degree in relevant field (preferred).
• Experience in the Human Services field.
• Computer proficiency.
• Valid driver’s license and reliable transportation.

Responsibilities

• Engage clients and complete initial case management assessment to determine eligibility and needs for services and housing.
• Assist clients with developing a housing plan and action steps, from obtaining documents and benefits to completing housing searches.
• Continuously engage clients in their housing goals and plan follow-up steps.
• Manage caseload by regularly reviewing progress and addressing barriers with a checklist.
