Job summary

Liberty Resources in New York seeks a Housing Specialist to support HUD and ESSHI programs across Onondaga and Madison counties. The role provides case management to help individuals achieve stable, independent housing, including financial independence and community integration.

Responsibilities include outreach, intake, housing search and placement, rental and utility assistance, and developing person-centered service plans in collaboration with clients and other providers.

Qualifications

• High school diploma and one year experience in a mental health or related setting.
• Bachelor's/Associates Degree preferred.
• Must possess a valid New York State driver’s license and reliable access to transportation.
• Must be able to use personal vehicle to transport others as required.

Responsibilities

• Provides community-based support for participants in the ESSHI Housing program.
• Outreach, assessments, intakes and referrals for housing options.
• Housing search, placement, and ongoing rental/utility assistance.
• Help participants develop daily living, transportation and community navigation skills.
• Develop linkages to improve access to community resources.
• Establish individualized service plans and coordinate with other providers.
• Provide housing placement assistance and ongoing rental support.
• Complete required documentation timely per program standards.

Education
