Job summary

Endeavors is seeking a Case Manager/Housing Navigator to provide individualized support to shelter residents, helping them access housing opportunities and stabilize into permanent housing.

You will build partnerships with landlords, conduct assessments, coordinate housing searches and required documents, and guide clients through leases, rights, and tenancy skills using trauma-informed, client-centered approaches. Travel as needed.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, Sociology or related field.
• Minimum 2 years of related experience in homeless services, housing navigation, or case management.
• Experience with Housing First philosophy and tenant rights a plus.
• Proficiency in English/Spanish (written and spoken) preferred.
• Ability to travel to various locations as needed.

Responsibilities

• Develop and maintain partnerships with landlords, property owners, housing authorities, healthcare networks, outreach teams, and community service providers.
• Assess housing providers and available units to ensure safe, appropriate housing placements aligning with client needs.
• Assist clients with housing searches, housing applications, lease coordination, and required documentation.
• Educate clients on tenant rights, lease agreements, housing responsibilities, and tenancy skills to support stability.
• Maintain a caseload of 20–30 clients with weekly meetings and timely documentation.
• Conduct intake assessments, eligibility screenings, and comprehensive evaluations of housing, financial, employment, educational, health, and supportive needs.
• Develop individualized housing and service plans with short- and long-term goals.
• Provide strength-based, trauma-informed, client-centered case management with motivational interviewing and de-escalation techniques.
• Facilitate referrals to employment resources, healthcare, mental health services, and benefits; submit HMIS referrals.
• Coordinate entry meetings, case staffing, trainings, and collaborative activities to support service delivery.
