Job description

Position Overview

**_The ideal candidate will have experience working with individuals facing homelessness, substance use issues, or serious mental illness (SMI). Key qualifications include familiarity with fair housing laws, community programs, and HUD-funded initiatives. HMIS (Homeless Management Information System) experience is a plus for this role._**

**_The position involves substantial travel, up to 75%, for community outreach and events in the following regions:_**

  • Elko County
  • Carson City, Douglas, Lyon, Storey, Churchill County
  • Nye County - Pahrump
Position Purpose

Responsible for supporting housing providers to comply with state and federal housing regulations and evidence-based practices related to housing. This includes assessing the needs of homeless individuals and families, assisting with obtaining and maintaining permanent housing, and collaborating with the Housing Administrator to improve housing placement and retention efforts. The role is remote and involves working with community partners, housing providers, internal care teams, and referral systems to coordinate services and support housing initiatives. Additionally, the role will support the implementation of Nevada Medicaid’s upcoming In Lieu of Services (ILOS) for Housing Supports & Services.

Key Responsibilities
  1. Collaborate with provider agencies to identify members qualified for housing assistance.
  2. Assist members with the housing application process and ensure timely submission of applications and supporting documents.
  3. Track and secure housing resources, and implement processes to help members secure and maintain housing and related services.
  4. Maintain an up-to-date housing database and document all services.
  5. Coordinate with providers to ensure timely transition of members to housing placement.
  6. Implement housing tools to prioritize placements.
  7. Facilitate processing of housing applications through collaboration with agencies and organizations.
  8. Assess the housing and family stability needs of residents and develop objectives and action plans for obtaining permanent housing.
  9. Perform other duties as assigned and ensure compliance with policies and standards.
Qualifications
  • Bachelor’s degree in a healthcare-related field or equivalent experience.
  • 3+ years of experience working with adults with SMI or in a healthcare setting.
  • Experience in non-profit housing services and knowledge of Continuum of Care, Permanent Supportive Housing, and Transitional Housing Programs.
  • Knowledge of fair housing laws, tenant rights, and SAMHSA evidence-based practices.
  • Current state driver’s license.
Compensation and Benefits

Pay Range: $57,700 - $80,800 per year. Benefits include health insurance, 401K and stock purchase plans, tuition reimbursement, paid time off and holidays, and flexible work arrangements (remote, hybrid, field, or office).
