Qualifications

• Knowledge of trauma-informed case management practices.

Responsibilities

• Perform Life Skill and Needs Assessments for new tenants to evaluate basic needs.
• Conduct 1-on-1 sessions with tenants to support service plan goals.
• Provide supportive services using recovery-oriented, trauma-informed, harm-reduction approaches.
• Educate tenants about self-help techniques and coping strategies.
• Maintain a supportive relationship with tenants through calls, texts, emails and in-person meetings.
• Document interactions and complete reports in the database.
