Qualifications

Responsibilities

• Conduct client interviews and assess needs; coordinate with service providers.
• Develop and monitor individualized service plans for clients.
• Assist in housing placement and coordinate with landlords.
• Maintain case records and ensure client confidentiality.
• Attend departmental and inter-program meetings.
