Job summary

Cornerstones in Reston is seeking a Case Management Coordinator for homeless prevention and rapid rehousing at the Embry Rucker Community Shelter. You will supervise case managers, coordinate referrals, and ensure trauma-informed, equity-focused service delivery.

The role emphasizes Housing First principles, collaboration with partners, HMIS data management, and meeting program outcomes. A bachelor’s degree or MSW plus management experience is required; bilingual abilities preferred.

Qualifications

• Bachelor’s degree in an applicable human services field or commensurate experience with homeless and/or at-risk populations to include supervisory/management experience.

Responsibilities

• Provides direct supervision for Homeless Prevention and Rapid Rehousing Case Managers.
• Carries a caseload of 12 households accessing Rapid Rehousing services.
• Manages state and county direct assistance funding streams.
• Provides regular case record reviews with case managers and ensures that case records meet contractual standards.
• Performs other related duties and responsibilities as required.
• Participates in multi-disciplinary meetings and community trainings.
• Works a flexible schedule that may include evenings and weekends.
• Supervises the practicum experiences for interns.
