Job summary

Connecticut Community Care is seeking a Housing Coordinator to work under the Money Follows the Person program. The role coordinates housing options, resources, and paperwork to ensure consumers secure safe, appropriate housing and that all duties support CCC’s mission.

You will collaborate with consumers, families, landlords, realtors, and state staff, identify at least three unit options, assist with viewing logistics and subsidy paperwork, and maintain up-to-date documentation in the state

Qualifications

  • Strong written and verbal communication.
  • Proficient computer skills, including Microsoft Office.
  • Able to stay organized in a fast-paced, dynamic environment.
  • Familiarity with needs of vulnerable populations (older adults, disabled).
  • Experience in housing service coordination is preferred.
  • Knowledge of CT housing resources and programs is a plus.

Responsibilities

  • Identify potential housing units in subsidized housing and present at least 3 options.
  • Coordinate with realtors, landlords, and others within the geographic area.
  • Assist clients with arranging transportation to view units and provide visual representations of units.
  • Complete and submit housing paperwork and subsidies documentation.
  • Identify and arrange modifications needed for accessibility and safety.
  • Maintain up-to-date documentation in the state system.
  • Attend meetings and support related projects as needed.

Summary

The Housing Coordinator, under the Money Follows the Person (MFP) program, works with consumers, families, providers, landlords, property owners, and other staff to coordinate, manage, and implement housing tools, resources, and inventories to ensure that consumers secure safe and appropriate housing. At CCC all duties are performed in a manner that fosters the achievement of the organization's mission.

Essential Functions
  • Works with realtors, landlords, and others to identify potential units in subsidized public and/or private housing, assuring MFP clients can be presented with at least 3 options.
  • Coordinates with realtors and landlords within geographic area assigned by the department.
  • Assists clients with arranging transportation to view units and/or provides clients with adequate visual representations of available units from which to choose
  • Completes and submits required housing paperwork.
  • Identifies the need for modifications to a rental unit and follows protocols.
  • Identifies and gains landlord approval regarding accessibility modifications required to make housing accessible and safe for clients
  • Ensures that consumers are aware of their role and responsibilities regarding applicable subsidies.
  • Assist clients after transition with issues pertaining to rental payments, landlord relationships, and local housing authorities.
  • Maintain up-to-date and accurate documentation in the state system.
  • Attends company and state meetings as required.
  • Performs other related projects or assignments as requested including photocopy work, data entry, typing, distribution of reports, etc.

Requirements

Education:

High School Diploma or equivalent; Bachelor’s Degree in Human Services or related field preferred.

Experience:

  • One to two years experience in housing service coordination for the aging and persons with disabilities is preferred.
  • Experience in real estate market and negotiation a plus
  • Knowledge of the Independent Living philosophy a plus.
  • Knowledge of community and housing resources in Connecticut, with the ability to understand and articulate multiple programs.
  • Proficient computer skills, including Microsoft Office.
  • Strong communication skills, both written and verbal.
  • Ability to keep organized and work effectively in a fast-paced, dynamic environment.
  • Familiarity with the needs of multiple vulnerable populations, especially older adults and persons with disabilities.
