Job summary

Concern Housing is seeking a Service Coordinator to link residents with community resources and provide daily living skills training, counseling, and support services. You will document activities, ensure program compliance, and advocate for clients in a non-profit housing setting.

The role requires a Bachelor's degree in Psychology, Rehabilitation, Social Work or related field and at least two years of experience in similar settings.

Qualifications

• Bachelor's degree in psychology, rehabilitation, social work or related field.
• Minimum 2 years experience in a similar setting or working with people with mental illnesses.

Responsibilities

• Facilitate residents’ linkage with community services (case management, clinical services, day programs, etc.).
• Develop quarterly goal plans and conduct weekly skill-building meetings; document client records.
• Provide individual counseling and support services.
• Assist with housing services and maintenance issues as needed.
• Deliver daily living skills training (medication monitoring, cleaning, money management, etc.).
• Perform monthly fire safety inspections and participate in supervision meetings.
• Maintain compliance with agency documents and trainings.
