Job summary

Samaritan Daytop Village, Inc. is seeking a Case Manager to work with adults and families experiencing homelessness in Elmhurst, Queens.

You will interview clients, assess needs, and collaboratively develop Independent Living Plans, coordinating referrals to housing, healthcare, education, and legal services. This full-time, non-exempt role pays $25.93/hour (about $47,210/year for 35 hours/week) with medical, dental, vision, 25 days PTO and retirement benefits.

Qualifications

• High School Diploma or equivalent required; higher degree preferred.
• 1 year of case management or social services experience helpful.
• Ability to build trust with diverse individuals.
• Clear communication in interviews, progress notes, and coordination.
• Maintain accurate records and meet reporting deadlines.

Responsibilities

• Build professional relationships with clients and maintain confidentiality.
• Review eligibility documents and explore non-shelter alternatives.
• Create and manage client files and records.
• Conduct intake interviews and psycho-social assessments.
• Develop and update Independent Living Plans with goals.
• Refer clients to permanent housing and community services.
• Support clients in overcoming barriers and advocating for themselves.
• Track client progress and document updates through regular contact.
• Follow up with clients and referral agencies to monitor outcomes.
• Provide data for required reports (weekly, monthly, quarterly, annual).
• Liaise with outside organizations for education, healthcare, housing, and legal matters.
