Job summary

Samaritan Daytop Village, Inc. is seeking a full-time Case Manager to work with individuals and families experiencing homelessness in New York City.

You will interview clients, assess needs, and collaboratively develop Independent Living Plans while coordinating referrals to health, education, and legal resources. The role emphasizes maintaining accurate records, meeting deadlines, and advocating for client access to housing.

Qualifications

• High School Diploma or equivalent is required; an Associate or Bachelor's in Human Services is strongly preferred.
• At least 1 year of case management or social services experience with homeless populations.
• Strong interpersonal and communication skills for interviews and documentation.
• Ability to maintain confidentiality and meet reporting deadlines.
• Familiarity with NYC housing, healthcare, legal, childcare, and education resources.
• Demonstrated organizational and multitasking abilities.

Responsibilities

• Build professional relationships with clients and maintain confidentiality
• Review client eligibility documents and explore non-shelter alternatives
• Create and manage client files and records
• Conduct intake interviews and psycho-social assessments
• Develop and update Independent Living Plans with short- and long-term goals
• Refer clients to permanent housing and community-based services
• Support clients in overcoming barriers and advocating for themselves
• Track client progress and document updates through regular contact
• Follow up with clients and referral agencies to monitor service outcomes
• Provide data for required reports (weekly, monthly, quarterly, annual)
• Act as a liaison with outside organizations for education, healthcare, childcare, housing, and legal matters
