Job summary

The Salvation Army Northern Division Harbor Light Center in Minneapolis is seeking a Case Manager to provide individualized, strength-based services to adults aiming for housing, employment, and self-sufficiency.

You will maintain participant records, recruit eligible participants, promote programs, and collaborate with community partners. A Bachelor's degree in a relevant field is required, with social work licensure preferred and background checks mandatory.

  • Maintain accurate participant records and ensure timely, compliant documentation.

Job description

TheSalvation Army Northern Division Harbor Light Center (HLC) and satellite locations is "Doing the Most Good" by providing shelter, housing, and holistic, person-centered, supportive services to over 300 single adults each night. HLC also offersworship services, pastoral care, and connections tocommunity partners.

The Case Manager provides individualized case management to supportadultparticipants in achieving housing, employment, and self-sufficiency goals. This roleis responsible formaintainingaccurateparticipant records, recruiting eligible participants, and promoting social services programs. Additionally, the Case Manager engages in community outreach, supports organizational initiatives, andfacilitateseducational opportunities to connect participants with valuable resources.

EssentialFunctions (approximate time spent)
  • Case Management(75%): Provideshort- andlong-term, strength-based, person-centered,andholisticcase managementforadult Develop and implementindividualizedservice plans,monitorprogress, and support participants in achievinggoals related tohousing, employment, and self-sufficiency.
  • Participant Information Management(10%):Maintainaccurateand up-to-date participant records in case files and databases. Ensure all case notes, assessments, and progress updates are documented ina timelyand compliant manner.
  • Participant Recruitment & Program Promotion(5%):Recruit eligible participants forhousing programsthrough outreach and referrals. Promote social services and corps programsto increase engagement and accessibility.
  • Community & Organizational Engagement(5%):Support and promote The Salvation Armyand HLCinitiatives by collaborating with colleagues andparticipatingin community events, forums, and outreach activities.
  • Educational Opportunities& Resource Development(5%):Facilitateindividual and group educational Contribute tothe maintenance of acatalog of local community resources and service providersto support participant success.

Full-Time, 40hrs/week

$23.99-$24.11/hour

Education and Experience
  • Bachelor’sdegreeinarelevant field; acombination of education and experience may be considered in place of a degree.
  • State board licensure isrequiredif the degree is in social work.
  • Previousexperience in social services isrequired.
  • Experience with long-term case management is preferred.
  • Mustsuccessfully pass all required background checks.
  • Avalid driver’s licenseis preferred, and candidatesmust meet the MVR requirements.
  • Strong interpersonal and communication skills with the ability to engage effectively with diverse populations.
  • Demonstrates integrity, professionalism, and discretion when handling confidential information.
  • Highly organized with strong attention to detail.
  • Proficient in Microsoft Office, databases, and general PC use.
WorkingConditions
  • Variety of work, sitting approximately 50% of the time with some prolonged standing, walking, and bending.
  • Must be able to lift to 20 pounds occasionally.
  • Work is conducted in a professional setting and is not eligible for hybrid, remote-friendlywork arrangement .
  • Requires clear and effective communication with colleagues, participants, and community members.
Supervisory Responsibilities

None.

Driving/TravelRequirements

If approved, drive agencyor personalvehicle locally to support the mission.
