Job summary

The Salvation Army Heritage Place in Omaha, NE is seeking a Case Manager for the Renaissance Village program. This full-time role runs Monday–Friday, 9:00 A.M.–4:30 P.M., with on-site responsibilities that may require driving to assist clients in securing housing and support services.

Responsibilities include intake and planning, resource referrals, advocacy with an interdisciplinary team, progress monitoring, and aftercare.

Qualifications

  • High school diploma or equivalent is required.
  • Experience in human services and ability to handle confidential matters.
  • Ability to access payroll/timekeeping data via web-based systems.
  • Good written and spoken communication; maintain professional relationships.

Responsibilities

  • Complete documentation and maintain client files (intake, plans, inventories).
  • Educate clients about community resources and refer as needed.
  • Advocate for clients with interdisciplinary team members.
  • Meet clients at their homes and monitor progress toward goals.
  • Provide aftercare services after program completion.
  • Maintain Homeless Management Information System records.
  • Participate in community meetings and task forces.
  • Provide on-call support as scheduled; driving is essential.

Summary/Primary Purpose:

The Renaissance Village is a transitional housing program, located at The Salvation Army Heritage Place. It offers short-term housing for men, women, couples and families. It is a 33-unit site based short term, 6 months, housing program. Case Managers assist residents with services, housing search and provide the support of residents to gain the skills and knowledge they need to thrive independently and to locate permanent housing. Most services provided occur on location, but Case Managers additionally may transport residents to community services. Hours have flexibility since clients reside onsite.

Essential Duties And Responsibilities
  • Complete all required documentation and maintain client files including intake, skills inventories and resources, individual service plans, and ongoing case management.
  • Educate clients about available community resources and entitlements and refer clients to appropriate community resources as needed.
  • Act as an advocate for the client with interdisciplinary team members.
  • Meet with clients in their homes and monitor progress related to their goals.
  • Provide aftercare services after successful completion of the program.
  • Follow The Salvation Army and program policies, procedures and emergency procedures.
  • Maintain and update client documentation including the required Homeless Management Information System.
  • Participate in assigned community meetings and task force meetings.
  • Provide on call support as scheduled on a rotating basis.
  • Driving is an essential aspect of the position.
  • All other duties as assigned.
Supervisory Responsibilities

Education and/or Experience:

High school diploma or equivalent required with at least one of the following: two years lived experience; two years direct care experience in a human services field; two years of training in a human services field; or, a bachelor’s degree in a human services field.

Qualifications:
  • Ability to be flexible and able to work on multiple projects or tasks simultaneously
  • The ability to access payroll, timekeeping and personal data via a web-based system.
  • Administrative skills to include utilization of standard office equipment, data entry, 10-key, attention to detail and filing.
  • Good communication skills both written and spoken, and ability to maintain effective working relationships.
  • Demonstrated ability to handle confidential matters.
  • Sensitivity to the service population’s cultural and socioeconomic characteristics.
Other Qualifications:

Must pass all applicable background checks. Must possess a valid driver's license from the state in which you reside. Must be approved through The Salvation Army Fleet Safety Program to drive either a Salvation Army or personal vehicle on Salvation Army business. Must successfully complete Safe from Harm training within 90 days of hire, as established by The Salvation Army.

Certificates, Licenses, Registrations:

Must complete CPR/First Aid training upon hire and maintain active certification for the duration of employment. Must complete Non-violent Crisis Intervention (CPI) training upon hire and maintain certification for the duration of employment. These trainings and any others required for employment will be provided by the employer. Must secure 12 in-service hours annually.

Working Conditions:

This position may require working in outdoor weather conditions. The noise level in the work environment is usually moderate.
