Job summary

Cornerstone is seeking a Housing Specialist to support individuals with ACT in securing safe, affordable housing and navigating benefits. The role emphasizes active outreach, individualized care coordination, and administrative support within a collaborative team.

Qualifications include a Bachelor’s degree with preferred case management experience and a valid driver’s license. The position is full-time, in-person, offering generous vacation, sick leave, holidays, and health benefits after 30

Qualifications

Responsibilities

  • Support clients with finding and maintaining safe and affordable housing.
  • Provide ongoing assistance to help clients apply for, understand and manage their benefits.
  • Ensure maximum acquisition of entitlements and other forms of assistance for all clients served by the ACT team.
  • Provide operational administrative support to the ACT team.
  • Assist clients in maintaining employment by frequent visits to the work site.

Our robust training program allows staff members to develop and grow in the position and provides the basis for upward mobility within the agency.Responsibilities of the Housing Specialist with the ACT TeamSupports clients with finding and maintaining safe and affordable housingProvides ongoing assistance to help clients apply for, understand and manage their benefitsEnsure maximum acquisition of entitlements and other forms of assistance for all clients served by the ACT teamProvide operational administrative support to the ACT team.Assists clients in maintaining employment by frequent visits to the work siteQualifications of the Housing Specialist with the ACT TeamBachelor’s degree and one (1) year of case management experience preferredExperience working with the population of serious mental illness in the capacity of case management, integrated care coordination, and medical office organization strongly preferredDriver’s license, proof of auto insurance, and access to a vehicle for work purposes.
