The ideal candidate has a Bachelor’s degree in social work or related field, along with relevant case management experience, ensuring effective support for vulnerable populations.

Qualifications

• 3-5 years of case management experience.
• Able to work independently with minimal supervision.
• Experience in initiating support services.

Responsibilities

• Assist health plan in identifying members for housing support.
• Coordinate with housing resources and agencies.
• Oversee housing assessments and ensure care continuity.

Skills

Case management experience in community homeless housing

Experience with diverse populations

Intermediate skills in Microsoft Programs

Time management and organizational skills

Education
