Job summary

RIM Recruitment Professionals is seeking a Community Supports Intake and Enrollment Coordinator in Los Angeles. You will engage referred members, guide enrollment, and coordinate transitions to housing navigation services, requiring clear communication and meticulous documentation both over the phone and in the field.

You will conduct high-volume outreach, verify Medi-Cal/L.A. Care eligibility, complete intake documents, and hand off to a Housing Navigator once enrollment is complete, while

Qualifications

• 1–2+ years in healthcare intake or enrollment.
• Experience with CalAIM Community Supports, outreach, or case management.
• Familiar with electronic health records and Google Workspace.
• Spanish fluency is a strong plus.
• Valid California driver's license and reliable transportation for field work.

Responsibilities

• Contact referred members via calls, texts, and field outreach.
• Verify Medi-Cal and L.A. Care eligibility during intake and enrollment.
• Document outreach and intake notes in HMIS or records.
• Coordinate with Housing Navigator and team leads for referrals and support.
• Maintain timely enrollment totals and participate in team meetings.
