Job summary

Connecticut Community Care seeks a Housing Coordinator to support the Money Follows the Person (MFP) program, coordinating housing options and resources for consumers, families, and providers.

The role involves partnering with realtors and landlords, arranging transportation, handling paperwork, and ensuring accessibility and subsidies are understood by clients. Strong organizational skills are essential.

Qualifications

  • High School Diploma or equivalent; Bachelor’s Degree in Human Services or related field preferred.
  • One to two years experience in housing service coordination for aging and disabled is preferred.
  • Real estate market knowledge and negotiation is a plus.
  • Familiarity with Independent Living philosophy a plus.
  • Knowledge of Connecticut housing resources; ability to articulate programs.
  • Proficient computer skills, including Microsoft Office; strong communication skills.

Responsibilities

  • Identify potential housing units in subsidized public/private housing with at least 3 options.
  • Coordinate with realtors/landlords within the assigned geographic area.
  • Assist clients with transportation to view units and provide visual representations of units.
  • Complete and submit required housing paperwork; manage modifications and approvals.
  • Maintain up-to-date documentation in the state system; attend meetings as required.

Education

High School Diploma or equivalent
Bachelor’s Degree in Human Services or related field
