Job summary

Qualifications

• 1+ years of clinical experience in family, geriatric, or home health preferred.
• Proficiency with electronic medical records (EMR).
• Ability to transport equipment and navigate stairs during home visits.
• Proven communication skills with geriatric or Medicare populations.

Responsibilities

• Conduct in-home assessments including history, vitals, and physical exams.
• Identify diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address gaps in care.
• Educate patients on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
