Job summary

Sample Supports seeks caring independent contractors to provide residential support to adults with intellectual and developmental disabilities under Colorado HCBS waivers. Opportunities include Host Home Providers and Family Caregivers.

Providers set their own schedules, complete required training and documentation, and receive ongoing support, oversight, and 24-hour on-call assistance from Sample Supports.

Qualifications

• Must be at least 18 years old.
• Must pass required criminal background and background checks.
• Must have a valid driver’s license and reliable transportation.
• Ability to transport the individual safely when transportation is part of the service plan.
• Ability to maintain privacy and confidentiality according to requirements.

Responsibilities

• Provide a safe, respectful home environment.
• Deliver services per person-centered plans and HCBS protocols.
• Support the individual’s dignity, rights, and independence.
• Complete electronic documentation accurately and on time.
• Coordinate with Sample Supports team and family members as needed.
• Maintain professional boundaries and training requirements.
