• Two years of experience in Health and Human Services or related field.
• Preference for group home or supported experience.
• A licensed/registered nurse is required for those deemed medically fragile.
• Be willing to share home with person supported.

Responsibilities

• Provide 24-hour care and supervision in a home setting.
• Offer healthy meals, transportation, and daily living support.
• Assist in developing programs and completing assessments per the participant's plan.
• Attend IPP meetings and complete required paperwork.
