Job summary

Sample Supports in Colorado is seeking caring independent contractors to provide individualized residential support to adults with IDD under Colorado HCBS waivers.

Provider roles include Host Home Providers and Family Caregivers who work as independent contractors, manage their own schedules, and deliver person-centered supports within the home and community context.

Training and on-call support are provided to assist with documentation, safety, compliance, and successful placement outcomes.

Qualifications

• Must be at least 18 years old.
• Must pass required criminal background checks.
• Must pass Child and Adult Protection Services checks.
• Must have valid driver’s license.
• Must have dependable transportation and meet MVR requirements.
• Must transport the individual safely when transportation is part of the service plan.
• Must provide a private bedroom unless approved otherwise.
• Must complete electronic documentation accurately and on time.
• Must complete required training and certifications.
• Must be willing to perform Nonviolent Crisis Intervention techniques when required.
• Must meet the individualized physical demands of the placement, which may include lifting or assisting with transfers.
• Must maintain reliable communication and respond to urgent situations.
• Must collaborate with the individual, family or guardian, case management agency, and Sample Supports team.
• Knowledge of IDD, mental health needs, and trauma-informed care is preferred but not required.
• All adult household members must complete required screening and training when applicable.

Responsibilities

• Provide a safe, stable, respectful home environment.
• Deliver services per the individual’s person-centered service plan and HCBS guidelines.
• Support dignity, privacy, rights, choice, independence, and community involvement.
• Assist with activities of daily living as needed.
• Coordinate transportation to appointments and activities.
• Complete and maintain electronic documentation for all services provided.
• Communicate regularly with Sample Supports and the individual’s team.
