Job summary

Hands of Heartland Inc. in Sioux City, IA is seeking a Host Home Provider to welcome and support a person with developmental disabilities in your own home, following the Iowa Medicaid waiver program.

You will assist with daily living activities, meal planning, transportation, and program implementation per the Individual Support Plan. Ideal candidates are 18+, have a valid driver’s license and car insurance, pass background checks, and ideally at least one year of relevant experience.

Responsibilities

Hands of Heartland is looking for compassionate and caring individuals to share their home with a person with developmental disabilities as a Host Home Provider. For this position, there should be no children in the home.

What You’ll Do

Provide supports to a person with a developmental disability in your home to ensure their wants and needs are met in compliance with the Iowa Medicaid waiver program. This can include assisting with activities of daily living, meal planning and preparation, providing transportation, assisting with medications, and implementing programs in accordance with the persons Individual Support Plan.
