Job summary

Hands of Heartland Inc. in Sioux City, IA is seeking a Host Home Provider to welcome and support a person with developmental disabilities in your own home, following the Iowa Medicaid waiver program.

You will assist with daily living activities, meal planning, transportation, and program implementation per the Individual Support Plan. Ideal candidates are 18+, have a valid driver’s license and car insurance, pass background checks, and ideally at least one year of relevant experience.

Qualifications

• Must be 18 years old and able to pass background checks.
• Must have a valid driver’s license and car insurance.
• Prefer at least one year of experience with individuals with developmental disabilities.

Responsibilities

• Provide supports to a person with a developmental disability in your home to meet wants and needs per the Iowa Medicaid waiver program.
• Assist with activities of daily living, meal planning and transportation as required.
• Implement programs in accordance with the person’s Individual Support Plan.
