Home Provider / Shared Living Provider

We are currently seeking a home provider in Forest City, IA / Winnebago County. This role offers a meaningful work opportunity in your home with an average annual income of $80,000 (federally tax‑free) plus room & board. Candidates must reside in Iowa and own or rent a home.

As a Home Provider, you will provide 24‑hour support to an individual with intellectual disabilities, welcoming them into your home and assisting with daily needs. Your responsibilities include maintaining a safe and healthy environment, mentoring, and helping with daily living tasks. Duties vary from minimal support (budgeting, medication, transportation, activities of daily living) to full support (dressing, toileting, feeding, positive behavioral support). The income is commensurate with the level of support provided.

Responsibilities

• Provide support for tasks such as cooking, cleaning, laundry, budgeting, decision‑making.
• Help with skill acquisition.
• Administer medication and attend medical appointments.
• Support overall health and well‑being.
• Document daily activities via web‑based case management application (THERAP).
• Provide fun and leisure experiences, community inclusion, integration, and socialization.
• Comply with all state/agency policies and procedures.
• Communicate effectively with team members.
• Support physical needs of the individual served.
• Provide transportation.

Possible Additional Responsibilities (depending on preference)

• Assistance with bathing, toileting, feeding, behavioral support.
• Support for medical protocols such as seizures, falls, feeding tubes.
• Care for individuals with complex physical/medical needs (hearing, vision impairment, non‑ambulatory).
• Potential support of behavioral issues.
