Job summary

Hands of Heartland, Inc. is seeking compassionate individuals to share their home with a person with developmental disabilities as a Host Home Provider in Sioux City, IA.

This is a full-time opportunity. You will provide supports to meet daily needs, assist with activities of daily living, meal planning, transportation, and medication support, and implement programs aligned with the person’s Individual Support Plan.

Qualifications

• Must be 18 years old.
• Must have a valid driver's license and car insurance.
• Must be able to pass all background checks.
• Prefer at least one year experience working with individuals with developmental disabilities.

Responsibilities

• Provide supports to a person with a developmental disability in your home to meet wants and needs per the Iowa Medicaid waiver.
• Assist with activities of daily living, meal planning and preparation, transportation, and medication support.
• Implement programs in accordance with the person’s Individual Support Plan.

Hands of Heartland is looking for compassionate and caring individuals to share their home with a person with developmental disabilities as a Host Home Provider. For this position, there should be no children in the home.

What You’ll Do: Provide supports to a person with a developmental disability in your home to ensure their wants and needs are met in compliance with the Iowa Medicaid waiver program. This can include assisting with activities of daily living, meal planning and preparation, providing transportation, assisting with medications, and implementing programs in accordance with the persons Individual Support Plan.
