Job summary

Hands of Heartland is seeking compassionate individuals to become Host Home Providers in Des Moines, IA. You will welcome a person with developmental disabilities into your home and provide supports to meet daily needs in alignment with the Iowa Medicaid waiver and the person’s Support Plan.

Requirements include being 18+, having a valid driver's license and car insurance, passing background checks, and ideally 1 year of relevant experience.

Qualifications

• Must be 18 years old.
• Must have a valid driver’s license and car insurance.
• Must be able to pass all background checks.
• Prefer at least one year experience working with individuals with developmental disabilities.

Responsibilities

• Provide supports to a person with a developmental disability in your home to ensure their wants and needs are met in compliance with the Iowa Medicaid waiver program.
• This can include assisting with activities of daily living, meal planning and preparation, providing transportation, assisting with medications, and implementing programs in accordance with the persons Individual Support Plan.

Hands of Heartland is looking for compassionate and caring individuals to share their home with a person with developmental disabilities as a Host Home Provider. For this position, there should be no children in the home.
