Job summary

Integrated Life Choices seeks a dedicated professional to develop, implement, and train habilitation programs within Host Home services. You will coordinate residential support, maintain program progress, and liaise with service coordinators, guardians, and medical professionals to ensure high-quality outcomes.

Responsibilities include onboarding new providers, ensuring contract compliance, conducting monthly reviews, and managing on-site visits and quarterly trainings.

Qualifications

  • Bachelor’s degree or higher preferred and relevant experience.
  • 2–4 years related experience or an equivalent combination of education and experience.
  • Ability to pass required background checks and complete ILC training.
  • Valid driver’s license and CPR/First Aid, QMAP, ESI Prevention certificates.

Responsibilities

  • Develops and implements habilitation programs for Host Home services.
  • Coordinates residential habilitation and communications with guardians, professionals, and service coordination.
  • Oversees contract compliance, audits monthly progress, and coordinates quarterly trainings.
  • Performs on-call duties and manages on-site home visits to ensure quality environments.
  • Handles onboarding of providers, billing accuracy, and SS representative payee duties when needed.
  • Maintains documentation, reviews Therap and related reports, and attends required meetings.

We hope you will consider joining us as an employee owner.Summary: Responsible for the development, implementation and training of habilitation programs in Host Home (HH) supported by ILC. Coordinates residential habilitation services of individuals moving in and living in HH services by performing the following duties.Essential Functions & Responsibilities include the following (other duties may be assigned):Prepares and understands the ISP process.Reads and follows up on Therap GERs, SComms, T-Logs and email communication.Communicates with service coordination, HHP, guardians and medical professionals on all pertinent information.Acts as a liaison between state, ILC and HHP.Develops, implements and maintains habilitation programs.Completes monthly review of program progress (revised as needed) and Therap monthly reviewsEnsures the development of and monitoring of Hopes and DreamsAttends weekly L10 meetings and all necessary meetingsActivity attends and participates in welcoming committee meetings and follows up on referrals per ILC expectations.Performs Management of Personal Funds and SS Representative Payee duties including communicates with social security office to become representative payee.Communicates with and has knowledge of financial institutions.Participates in on-boarding process of provider including but not limited to reviewing application, interviewing, reviewing references, ensuring pre-screening survey is complete and to standards, ensures all paperwork is properly completed and maintained, and training is complete before intake meetings.Conducts monthly home visits to Host homes to ensure environment is suitable for the individual being supported and that programs are being implemented properly and monthly electronic reviews that are both added to QIS.Oversees HHP for contract compliance and completes breach of contract corrective when issues arise.Conducts environmental on-site visits for prospective HHP and annually.Conducts meet and greet with individual, service coordination and guardian with potential HHP.Facilitates contract signing and ensures all information is complete including belongings, inventory and pictures, and all books are put together.Conducts HHP quarterly trainings and meetings.Performs on-call duties for all HHP caseloads.Remains on-call on weekends and/or holidays as assigned.Ensures billing is accurate and completed bi-monthly per ILC timelines.Files as needed.Performs all other duties assigned.Qualifications:To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to write speeches and articles for publication that conform to prescribed style and format.Math Ability:Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry.Reasoning Ability:Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
