Job summary

CHR's Behavioral Health Home (BHH) program in Manchester, CT is seeking a BHH Specialist for a full-time position (40 hours/week). The role focuses on whole-person care for adults with complex medical and mental health needs, with a schedule Monday–Friday 8:00 AM–4:30 PM or 8:30 AM–5:00 PM.

You will provide comprehensive care management, care coordination, health promotion, and patient-family support, liaising with primary nurses, and connecting clients to community services.

Qualifications

  • BA in human services required.
  • Two years experience in mental health setting; primary care experience preferred.
  • Bilingual candidates strongly preferred.

Responsibilities

  • Provide Comprehensive Care Management and Care Coordination.
  • Promote health and wellness; support patients and families.
  • Coordinate with BHH team to improve care outcomes.
  • Assist with access to medical, housing, transportation, and community services.
  • Document activities in compliance with program standards.

Duties & Responsibilities
  • BHH specialists provide
    • Comprehensive Care Management
    • Care Coordination
    • Health Promotion
    • Comprehensive Transitional Care
    • Patient and Family Support
    • Referral to Community Support Services
  • Collaborates with BHH Primary nurse care manager and other BHH team members to provide improved medical and behavioral health care coordination and outcomes for BHH enrollees.
  • Works to reduce health care disparities whenever possible
  • Demonstrates competency in age related or specialty issues and developmental needs for each population served.
  • Assists in obtaining access to medical and wellness services including tobacco cessation intervention services, benefits, housing, treatment, legal services, medical services, education and employment, as needed.
  • Assisting with money management and other activities of daily living
  • Assisting patients with accessing necessary health care including transportation
  • May provide client transportation as needed.
  • Provides high level of collaboration with medical providers to ensure medical follow up including PCP & specialist visits, labs, test and procedures to manage chronic health conditions.
  • Participates in hospital discharge process to promote highly coordinated care transitions for behavioral health and or medical admissions.
  • Provides wellness and health-promoting lifestyle interventions such as, but not limited to nutritional counseling, smoking prevention and cessation, obesity reduction, increasing physical activity.
  • Provides crisis intervention and support as clinically indicated.
  • Works as liaison between agency and state and private hospitals, other community-based services, family members and significant others.
  • Maintains documentation in compliance with agency and program standards.
  • Participates in staff meetings, supervision, training and other meetings as assigned.
  • Additional duties as required.
QUALIFICATIONS:

Education: BA in human services required.

Experience: Two years experience in mental health setting. Experience working with primary care providers preferred.

Bilingual candidates strongly preferred.

Licensure/Certification/Registration: Valid driver’s license. CPR/First Aid Certification within 6 months of hire.

COMPENSATION*

$19.28/hour min., and up, commensurate upon experience.

*Actual rates are determined at the time of offer and are based off relevant experience, as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
