Job summary

Community Health Resources in Hartford, CT seeks a Program Director for the Credible Messenger program. This Full-Time role leads a multidisciplinary team, oversees service delivery, and ensures compliance with funders and licensing standards.

You will manage staff, budgets, and quality-improvement initiatives, collaborating with partners to promote engagement and timely access to services for youth and families.

Qualifications

  • Bachelor’s degree preferred in a related field or equivalent experience.
  • Minimum 3 years of relevant work experience; 2 years supervisory experience in behavioral health preferred.

  • Provides direction and leadership within the Team.
  • Ensures clients receive services aligned with treatment needs.
  • Directs crisis intervention when required.
  • Monitors meetings, admission, discharge criteria.
  • Reviews caseloads for appropriate service utilization.

JOB TITLE: Program Director

EMPLOYMENT TYPE: Full-Time, 40 hours/week

SCHEDULED HOURS: Monday-Friday 9:00AM-5:00PM

PROGRAM/LOCATION: Credible Messenger, Hartford, CT

ABOUT THE PROGRAM: Credible Messenger Program

The Credible Messenger program provides community-based care coordination, engagement, and support to youth referred by the Court Support Services Division/Juvenile Court Support Services Division (CSSD/JBCSSD). The program acts as a liaison between CHR, CSSD/JBCSSD, clients, families, and community partners to support timely access to services, reduce barriers to treatment, and promote successful engagement in care. The program aims to assists youth and families with identified needs related to behavioral health, substance use, medical care, housing, education, employment, legal matters, transportation, benefits, family support, and other activities of daily living, while completing administrative, programmatic, triage, and documentation responsibilities in accordance with program and contract expectations.

Duties & Responsibilities
  • Provides direction and leadership within the Team.
  • Ensures clients receive services that are focused on presenting problems and clients’ request for treatment.
  • Directs crisis intervention efforts and intervenes as required.
  • Monitors client meetings, admission, continued stay and discharge criteria.
  • Reviews caseloads regularly to ensure appropriate utilization of services.
  • Oversees hiring, training, and performance management of staff within the program.
  • Facilitates team meetings.
  • Participates in internal and external meetings as needed by program and agency.
  • Ensures client, program and agency documentation is completed in a timely manner and according to program, agency, funder and accreditation/licensing standards.
  • Acts as primary point person for program and provides back up coverage as needed by program and agency.
  • Ensures proper maintenance of vehicles/if applicable.
  • Will collaborate across teams and divisions to improve quality, workflow, and communication to improve efficiency and general outcomes
  • Ensures timely completion of reports.
  • Oversees completion of chart audits.
  • Ensures completion of insurance authorization for services rendered as necessary.
  • Develop and executes marketing plan for program as needed
  • On call coverage during and after program hours as assigned.
  • Ensures proper maintenance of vehicles/if applicable.
  • Participates in committees to improve agency service delivery and quality improvement.
  • Responsible for program budgets and productivity expectations
  • Ensures compliance with regulating bodies.
  • Additional duties as required.
QUALIFICATIONS

Education: Bachelor’s degree preferred but relevant life experience can count with the possibility of a CSSD waiver.

Experience: A minimum of 3 years’ experience working with identified population. Two years of supervisory experience in a behavioral health setting preferred.

COMPENSATION*

Starting at $65,875/yr min., and up commensurate upon experience.

  • Actual rates are determined at the time of offer and are based off of relevant experience, as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
